Insurance/Payment

We participate with most major insurance companies. Please check with your insurance company's customer provider number provided on your personal insurance card.

Always remember to go to the hospitals, lab and physicians that participate with your insurance carrier to avoid any unnecessary expense to you.

This does not mean you can not seek professional medical care outside your listed participating providers, only that you will be responsible for those charges not approved and paid by your insurance carrier.

Please contact your PCP (primary care physician) for the proper referral.
